We know that every caregiving situation is unique, so we customize your Care Plan to match the specifics of your medical, financial, and socio/cultural circumstances.

Every case begins with an initial assessment by an experienced Registered Nurse. The assessment is multi-faceted and looks into many things, such as the client’s physical and mental health and also any social dynamics that need to be considered in the delivery of care. 

The assessment is then used to create a Care Plan and to assign an in-home Caregiver that best matches the patient based on the issues identified. This Caregiver will be responsible for day-to-day care delivery.

As part of Gevity’s commitment to quality care, the RN will do a post assessment follow-up by phone in 30-days with the patient and or family to check in and to determine if another visit is necessary. The RN will also make a mandatory second visit after 60 days.
